The form is not monitored for emergencies. Do not use it for urgent medical help or to send symptoms, medical history, records or patient photographs.
What you share
The form asks for your name, phone number and area of care. A preferred date is optional. Your area of care and preferred date, together with your contact details, can reveal sensitive information about an interest in healthcare.
We also record a request reference, submission time, consent and the request’s progress status so staff can organize and respond to your enquiry. The form does not accept file uploads or medical records.
Why the clinic uses it
The clinic uses these details to reply to your request, discuss consultation availability and coordinate the next step. Submitting the form does not subscribe you to marketing messages.
Only share contact details you are comfortable using for a reply. If you prefer, you can contact the clinic directly by phone or through the official WhatsApp links instead of submitting the form.
Storage and access
The website uses Cloudflare Workers to process requests and Cloudflare D1 to store them. The administrative workspace restricts access to authorized clinic staff. These service providers process information as part of operating the website.
Clinic staff may export request details when organizing follow-up. The clinic is responsible for restricting access to any exported copies and reviewing them when information is no longer needed.
Cookies, language and external links
The administrator workspace uses a necessary session cookie to keep authorized staff signed in. The site stores your preferred language in your browser’s local storage. This website does not use advertising trackers or analytics tools.
If you open Instagram, WhatsApp or Google Maps links, you leave this website. Those services handle information under their own privacy policies. Cloudflare may process technical information needed to deliver and protect this website.
Keeping information up to date
The clinic reviews how long request information is needed for responding and follow-up. No fixed retention period is stated in this notice. Contact the clinic to ask about a request, correct your details, or request removal of information, including any exported copies.
